The premise

Here’s the bit.

During a visit to his doctor, Derek learns about a new health clinic for gay men. He treats the suggestion as an outrageous assumption about his private life and launches into an increasingly theatrical protest. The doctor tries to explain, but Derek turns references to his mother and his own belongings into further reasons to be offended. He ends the appointment by collecting his bag and calling for his companion Leonard, leaving the doctor to absorb the encounter.
